Please bring the following items to your appointment:

• Insurance card
• Information regarding your medical and surgical history
• A list of all current prescription and/or over-the-counter medications, including dosage and frequency
• Most recent X-rays, imaging, or other test results related to your condition

Prior to your first appointment, please review, complete, and print the following forms:

• New Patient Form (fillable form)
• Patient History Form (fillable form)
  
• Notice of Privacy Practices
• Patient Responsibility Agreement
• Authorization to Release Medical Records
• Authorization for Use and Disclosure of Protected Health Information
• AUA Symptom Score
• Sexual Health Inventory for Men

Our physicians accept most major insurance plans; however, we recommend contacting our office prior to your appointment to confirm that your insurance provider is currently accepted. If your insurance company requires a referral prior to your visit, please obtain the referral from your primary care provider in advance.

Please note that co-payments and any charges not covered by your insurance provider are the responsibility of the patient or guardian.
